CLASS ONE OUTLINE: Prenatal Care

Your history as a homebirth midwife, childbirth educator in US; your current status as childbirth educator in France; plans to form nonprofit for childbirth/waterbirth education in France

Reiterate that this class is designed for all woman, regardless of where they choose to birth, although many of the issues you address will be specific to homebirth.

What is involved in proper prenatal care? What is needed from you to assist this unique being's development and safe arrival?

The baby experiences everything the mother experiences while in utero - physical, emotional, mental & spiritual
so we will briefly discuss many of the changes that take place in all these areas and how a mother can care for herself wholly

PHYSICAL CHANGES

  1. What happens to a woman's body during pregnancy
    1. Primary changes: uterus, cervix, vagina, breasts and pelvis
    2. Secondary changes: morning sickness, weight gain, heartburn, dizziness, fatigue, blood volume expansion, frequent urination, hemmorhoids, varicose veins, mental fog
  2. Fetal growth
    1. First trimester
    2. Second trimester HANDOUT
    3. Third trimester
  3. How to physically care for herself and baby during pregnancy
    1. Nutrition-lots of protein, vegetables, limit sweets, limit caffeine
    2. Exercise-walking, yoga, swimming
    3. Tisanes-raspberry and nettles, other herbs for specific complaints
    4. Kegels/Peri Massage - will be taught in third class, but mentioned now as well

EMOTIONAL CHANGES

  1. Explanation of hormonal changes and their effects on emotions
  2. Discuss the huge effect of her own birth and her mother's experience, how she has the ability to break old patterns and have a positive birth experience; Encourage women to ask their mothers about their birth experiences and feelings toward pregnancy
  3. Discuss the effect of one's relationship with intimacy and sexuality can have on birth, especially regarding abuse
  4. Discuss how one's relationship with her partner can affect her pregnancy; good stable relationship vs. unstable relationship or no relationship; the role of the father
  5. Discuss care and preparation for emotional changes and needs: honest assessment of feelings, discussion with loved ones, etc

MENTAL CHANGES

  1. Explain how some women experience changes in their memories, etc,
    during pregnancy - how all their energy is being used for developing the baby and not for thinking or mental activities
  2. Explain how confidence based on knowledge and certainty is key to a positive birth experience
  3. Discuss the power of fear, especially in hospital births; encourage women to educate themselves on typical hospital procedures
  4. Discuss care and preparation that will allow them to trust the body:educate themselves about birth, talk to other couples, write a birth plan

SPIRITUAL CHANGES

  1. Discuss how proper care prepares them to welcome a new soul into incarnation

TYPICAL PRENATAL CHECK

  1. Urine check for protein and glucose - what each is and what it means if either is present
  2. Palpation - explain the different possible positions of baby: anterior, posterior, vertex, breech, floating, etc; how many weeks they can expect the baby to turn vertex, engage, etc.
  3. Mom's blood pressure and pulse - importance and normal ranges; common causes for elevation, like anxiety, stress, fatigue, insufficient fluids, lying on her back
  4. Fetal heart tones - normal ranges and how many weeks first able to be heard; dangers of using Doppler in early weeks
  5. Fundal height- how centimeters often corresponds with weeks of gestation; how fundal height drops when baby engages; where fundal height is measured from
  6. Discuss labwork - hematocrit and hemoglobin - what they are and their importance
  7. Discuss ecographies
